This lesson plan template follows a gradual release model: you show, students try with help, then students work alone. It fits most subjects and grades.
Template structure
Objectives
One to three measurable outcomes beginning with an action verb.
Materials and prep
List everything you need, and what to set up beforehand.
Warm-up (5 min)
Activate prior knowledge with a quick question or task.
Direct instruction (10 to 15 min)
Teach the idea with a model and two examples.
Guided practice (10 min)
Students practice with your support. Check for understanding.
Independent practice (10 to 15 min)
Students work alone or in pairs. Prepare an extension task.
Assessment and wrap-up
Exit ticket or short quiz, and a preview of the next lesson.
